We are currently seeking a compassionate Licensed Vocational Nurse (LVN) for our South Bay territory. In this role, you will work independently in the field while staying connected to a dedicated Director of Nursing and back-office support team who handle the logistics so you can focus on the patient.
Requirements
- Current California Licensed Vocational Nurse (LVN) license.
- IV Certification and wound care/wound nursing experience required.
- Minimum one year of previous nursing experience in the medical field.
- Valid California Driver's License.
Nice To Haves
- Home Health experience is recommended but not required
Responsibilities
- Implement the plan of care initiated by the Registered Nurse.
- Provide skilled nursing care including wound care and medication administration.
- Educate patients and family members on diagnosis and care plans.
- Collaborate with the interdisciplinary team (RN, PT, OT, ST, MSW) and report identified needs to the Clinical Supervisor.
- Complete accurate and timely documentation in Kinnser on company-provided devices.
- Maintain effective communication with the back-office team to ensure seamless care coordination.
