About The Position
Health Link Home Health and Hospice is seeking a compassionate, organized, and team-oriented Full-Time Home Health LVN to serve patients throughout the South Bay area. This role involves collaborating closely with the Director of Nursing, clinical staff, and office team to ensure coordinated, high-quality care. Health Link emphasizes a supportive and collaborative work environment, with a dedicated back-office team and modern tools to assist field clinicians.
Requirements
- Minimum 1 year of nursing experience in a medical setting
- IV certification and experience with wound care
- Active California LVN license
- Valid California Driver’s License
Nice To Haves
- Home health experience recommended but not required
Responsibilities
- Implement the plan of care initiated by the Registered Nurse
- Provide accurate and timely documentation consistent with the plan of care
- Assess and educate patients and caregivers regarding diagnosis, care plans, and treatments
- Participate in coordination of home health services, reporting additional needs (HHA, PT, OT, ST, MSW) to the RN or Clinical Supervisor
- Use equipment and supplies effectively and responsibly
- Complete online charting in Kinnser using company-provided devices
- Follow all company policies and procedures
- Perform other duties as assigned
