About The Position

The Licensed Therapist (LICSW, LPC, or LMFT) is responsible for providing compassionate, evidence-based mental health services to clients within long-term care settings. This position plays a vital role in supporting the emotional, psychological, and behavioral well-being of individuals by delivering therapeutic interventions and collaborating with interdisciplinary teams to ensure coordinated, person-centered care.

Requirements

  • Must hold a LICSW, LPC, or LMFT license.
  • Ability to travel to assigned facilities as needed.
  • Ability to sit, stand, and move throughout the workday, including light lifting (<25 lbs).
  • Maintain professional appearance and demeanor in all settings.

Responsibilities

  • Conduct comprehensive initial assessments and develop individualized treatment plans.
  • Provide individual therapy sessions and occasional family therapy using clinically sound, evidence-based approaches.
  • Deliver therapy services within nursing homes and assisted living facilities.
  • Support clients experiencing a wide range of mental health concerns including depression, anxiety, medical issues, severe mental illness, substance abuse history, trauma, grief, and adjustment issues.
  • Maintain timely, accurate, and confidential documentation in the electronic medical record (EMR) via SOAP note format, in accordance with HIPAA and company policy.
  • Collaborate frequently with psychiatric providers, nursing staff, and facility leadership to ensure continuity of care.
  • Participate in monthly virtual team meetings and case consultations.
  • Provide crisis intervention and follow-up as needed to ensure client safety and stability.
  • Monitor client progress and adjust treatment plan goals and interventions as clinically indicated.
  • Engage in ongoing professional development and continuing education to maintain licensure and enhance clinical practice.
  • Adhere to all applicable state and federal laws, professional codes of ethics, and Remedy Behavioral Health policies.
  • Comply with HIPAA, Confidentiality, and Documentation standards.
  • Promptly report any compliance or safety concerns to leadership or the HIPAA Officer.
  • Participate in quality assurance, performance improvement, and compliance training initiatives.
