About The Position

Clinica Sierra Vista is seeking a Licensed Therapist IBH to join their team. This role operates within an integrated or collaborative care model in a Community Health Center, serving as an advanced psychotherapy position. The therapist will provide care to patients with mild to moderate behavioral health, substance use disorder, and co-occurring treatment needs. Responsibilities include warm hand-off consultations with primary care providers and patients, developing treatment plans with the patient and medical team, delivering brief psychotherapy and psychoeducation interventions, conducting assessments for care coordination, and coordinating psychiatric and medical services. The position requires meeting minimum productivity standards.

Requirements

Hold a valid license as a Licensed Clinical Social Worker (LCSW), Licensed Marriage & Family Therapist (LMFT), or a Licensed Clinical Psychologist.
Four years or more of psychotherapy experience, working with adults and families.
Familiarity with brief therapy models.
Demonstrate a basic understanding of the relationship between physical and mental wellbeing, including the risks and effects of substance use.
Ability to work independently and manage time and cases effectively.
Ability to turn in paperwork by deadlines.
Culturally competent and demonstrate ability to engage with patients of the community health center.
Learn and comply with Agency policies, procedures and ethical standards, HIPAA laws and guidelines, and the Professional Code of Ethics of the BBS of the State of California.
Ability to collaborate with Agency staff, other organizations or contacts.
Computer literate.
Excellent verbal and written clinical skills.
Current California Driver’s License.
Reliable access to an automobile with liability insurance.
Clearance by the Department of Justice prior to employment.
TB test prior to employment.
Possess and maintain current Basic Life Support (BLS) card, American Heart Association accredited required.
Adhere to Clinica Sierra Vista’s employee health/immunization requirements or provide a valid exemption request for subsequent approval.

Familiar with and able to practice consistent with Recovery principles in mental health care.
Familiar with and able to practice consistent with the treatment of Co-Occurring Disorders.
3-4 years of working in community mental health or in an integrated behavioral health program is preferred.

Responsibilities

Provide care to patients with Mild to Moderate behavioral health, substance use disorder and co-occurring treatment needs.
Provide warm hand-off consultations with primary care providers (PCP) and health center patients.
Develop treatment plans with the patient and medical team.
Deliver brief psychotherapy and psychoeducation interventions.
Provide assessments to appropriately identify and coordinate client’s care.
Coordinate psychiatric and medical services with the patient’s overall treatment plan.
Meet minimum productivity standards of the agency.
Work effectively in an interdisciplinary team approach with both paraprofessionals and professionals.
Maintain client dignity and confidentiality.
Attend regular meetings and articulate brief psychotherapy concepts for the cross learning benefit of the integrated team.
Perform other job-related duties as required.
