About The Position

A Licensed Social Worker (LSW) in a skilled nursing facility (SNF) provides comprehensive psychosocial assessments, care planning, and therapeutic interventions to support residents' mental and emotional well-being. They facilitate admissions/discharges, advocate for resident rights, and mediate between families and the interdisciplinary care team.

Requirements

  • Bachelor’s or Master’s degree in Social Work (BSW/MSW) from an accredited program.
  • Valid state licensure (LSW, LCSW, or equivalent).
  • Strong communication, documentation, and crisis management skills.

Nice To Haves

  • Previous experience in a nursing home, hospital, or geriatric care is preferred.

Responsibilities

  • Conduct comprehensive psychosocial assessments upon admission and develop individualized, 90-day, or annual care plans.
  • Coordinate with insurance managers, families, and community resources to manage safe discharges.
  • Ensure compliance with federal/state regulations, promote resident dignity, and protect resident rights.
  • Offer crisis intervention, counseling, and education to help residents adjust to long-term care.
  • Participate in daily rounds, care conferences, and team meetings to review clinical, emotional, and social needs.
