About The Position
Continuum Care Hospice is seeking a compassionate and experienced Licensed Social Worker (MSW) to join its team of professionals in Toledo, Ohio. The part-time Hospice Social Worker supports patients and families through end-of-life care, with an emphasis on dignity, family connection, and emotional peace. The role includes providing emotional support and crisis intervention, helping families with end-of-life planning and resource navigation, and serving as a connection between the clinical team and the home. Requirements
