About The Position

Ascension is seeking a Licensed Social Worker for their Family Health Center West in Nashville, TN. This full-time role in Primary Care focuses on empowering patients through diagnosis, developing custom care plans, championing collaboration with various agencies and families, and delivering versatile psychotherapy across a spectrum of needs, from crisis intervention to long-term therapy.

Requirements

• Master's degree in Social Work required.
• Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.
• Licensed Clinical Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date.

Nice To Haves

• Drivers License.
• 3 years of experience preferred.

Responsibilities

• Administer and interpret psychosocial assessments to diagnose and treat mental and behavioral health conditions.
• Develop and lead assessment-driven treatment plans.
• Partner with team managers, community agencies, and families to create a seamless circle of support and ensure long-term plan compliance.
• Provide psychotherapeutic care, including crisis intervention, short-term, and long-term therapy.
