Licensed Social Worker (Part time) Naples
About The Position
This is a part-time position for a Licensed Social Worker in Naples. Responsibilities include patient assessment and care planning, psychosocial assessments, and counseling. The role also involves completing various assessments such as KDQOL and Clinical Depression Screen, making referrals to outside agencies, and addressing patient needs related to transportation, health insurance, and medication coverage. Collaboration with billing is also a part of the role. Proficiency in written communication is essential for completing monthly social work notes, depression screenings, quarterly MSPQ's, and other required documentation. The Social Worker will also participate as a member of the Interdisciplinary Team to identify effective interventions and rehabilitation goals to improve patients' quality of life.
