Licensed Social Worker -Part Time
About The Position
This is a part-time position for a Licensed Social Worker. Responsibilities include patient assessment and care planning, counseling, psychosocial assessments, administering screenings such as KDQOL and Clinical Depression Screen, making referrals to outside agencies, addressing transportation needs, and assisting with health insurance and medication coverage. Collaboration with the billing department is also a key function. Proficiency in written communication is essential for completing monthly social work notes, depression screenings, quarterly MSPQ's, and other required documentation. The Social Worker will also participate as a member of the Interdisciplinary Team to identify effective interventions and rehabilitation goals aimed at improving patients' quality of life.
