About The Position

Summit County Public Health is seeking a full-time Licensed Social Worker (LSW) to join their Senior & Adult Services program. This role is crucial in promoting health equity for older adults in Summit County through wellness, prevention, healthcare access, and safety initiatives. The LSW will be responsible for assessing, planning, and implementing strategies to improve the health and independence of older adults in the community. The position operates Monday through Friday, with a standard 35-hour work week. Requirements

• Bachelor's degree in Social Work or related field.
• Active LSW license through the State of Ohio Counselor, Social Worker & Marriage & Family Therapist Board.
• Minimum of 3 years related work experience.
• Knowledge of resources and services in Summit County for individuals and families.
• Experience with multisystem, multi-need individuals and families, treatment planning, coordination of services, linking services, and record keeping.
• Valid Ohio driver's license, automobile availability and insurance coverage.

Responsibilities

• Provide case management through the GateKeeper program to support safe, independent living for older adults.
• Assess the specific needs of older adults to ensure tailored, independent-living support services.
• Provide BRI Care consultations to support individuals living with dementia and their family caregivers.
• Conduct targeted outreach for caregivers, focusing on effective communication, self-care, and available resources.
• Facilitate educational initiatives on brain health, fall prevention, and dementia capability.
• Partner with initiatives such as Dementia Friends Ohio to build a more age-friendly Summit County.
• Collaborate with local community groups and partners to expand service reach and support.
• Administer program grants while ensuring accurate and timely documentation for all services.
