About The Position
This role is for a Licensed Social Worker at a Family Health Clinic, specializing in Primary Care. The position is full-time, with day shifts. The social worker will design person-centered care plans, act as a patient advocate, serve as a clinical subject matter expert for the care team, and drive care excellence through collaboration with supervisors and transition teams.
Requirements
- Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.
- Licensed Clinical Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date.
- Master's degree in Social Work required.

Responsibilities
- Design comprehensive transition plans that bridge the gap between hospital and home, ensuring every step reflects the patient's unique needs and goals.
- Proactively identify and dismantle barriers to care, ensuring patients have the resources and support systems they deserve.
- Serve as the go-to resource for the care team on complex issues, including crisis intervention, neglect, and adjustment needs.
- Maintain a continuous, collaborative pulse with supervisors and transition teams to refine health plans and ensure seamless implementation.
