About The Position

The Licensed Social Worker MSW will initiate direct patient care services, family and patient counseling discharge planning, crisis intervention and consultation, coordinate referrals and promote educational programs. Have access to confidential employer, patient and financial information. Responsible for making independent decisions and in collaboration with team members. Provides direct patient care to all ages through the continuum of care.

Requirements

• Master’s Degree
• Current Social Work license from the state of Ohio Counselor and Social Work Board
• 1 year experience in social work

Nice To Haves

• Previous home care experience preferred
• Experience in discharge planning for Care Management
