About The Position

Senior Care Options by Parker provides customized Geriatric Care Management services as well as Behavioral Health services by licensed social workers to individuals in their homes and communities across Middlesex, Monmouth, and Ocean Counties. Program participants of both programs benefit from services offered in their own environment by our dedicated care partners who are committed to meeting their needs. Services will help support participants in maintaining independence in their own environment. This role is for an LSW in Home Counselor for Monmouth County, focusing on in-home geriatric counseling. The position requires a minimum of an 8-client caseload, with free clinical supervision provided for those meeting this caseload requirement. Service areas include Matawan, Middletown, Neptune, Asbury Park, Long Branch, and Ocean.

Requirements

• Master's degree required.
• Valid LSW in the state of NJ required.
• NJ driver's license in good standing required.
• Flu vaccine required during flu season.

Nice To Haves

• Two (2) years of counseling experience preferred.
• EHR knowledge preferred.

Responsibilities
