About The Position

PrimeCare Medical is seeking a Licensed Professional Counselor (LPC) or Licensed Social Worker (LMSW or LCSW) to join their medical department as a Behavioral Health Professional at the Berks County Jail in Leesport, PA. This role involves providing 40 hours of on-site care, with a rotating schedule of day and evening shifts, and required Saturday rotation.

Requirements

• Must be licensed as a LPC, LSW, or LCSW in the Commonwealth of Pennsylvania.
• Must hold a Master’s degree or greater in Social Work, Psychology, or related Human Service field with a concentration in clinical course work, training and/or experience.
• Minimum of two years of experience.

Responsibilities

• Conduct clinical assessments based on referrals and self-referrals, including documentation of findings, mental status examination, case conceptualization, diagnosis, and treatment plan. Make referrals to psychiatry when appropriate.
• Conduct risk assessments utilizing structured interviews and risk assessment tools. Manage inmates at risk for suicide and/or engaging in self-harm according to existing protocols and current policy and procedures.
• Serve as the PrimeCare Medical (PCM) mental health liaison for mental health services outside the facility.
• Facilitate admission and placement of inmates/patients to forensic hospitals when necessary.
• Monitor all mental health services provided.
• Monitor clinical services to ensure standards of care are in accordance with NCCHC and accepted psychiatric/psychological standards.
• Serve as a member of the Quality Assurance Committee.
• Develop and conduct in-service education programs according to the annual in-service calendar.
• Collect data and prepare monthly reports reflecting performance.
