About The Position

PrimeCare Medical is seeking a Licensed Professional Counselor (LPC) or Licensed Social Worker (LMSW or LCSW) to serve as a Behavioral Health Professional. This role involves traveling to and working within multiple correctional facilities, including Westmoreland County Jail, Somerset County Jail, Washington County Jail, and Cambria County Jail. The position is full-time, 40 hours per week, with a standard Monday to Friday schedule during business hours (8:00 am - 4:30 pm). The role requires the ability to travel and provide services across all listed facilities.

Requirements

• Must be licensed as a LPC, LSW, or LCSW in the Commonwealth of Pennsylvania.
• Must hold a Master’s degree or greater in Social Work, Psychology, or related Human Service field with a concentration in clinical coursework, training and/or experience.
• Minimum of two years of experience.

Responsibilities

• Conduct clinical assessments based on referrals and self-referrals, including documentation of findings, mental status examination, case conceptualization, diagnosis, and treatment plan.
• Make referrals to psychiatry when appropriate.
• Conduct risk assessments using structured interviews and risk assessment tools.
• Manage inmates at risk for suicide and/or engaging in self-harm according to established protocols and policies.
• Serve as the PrimeCare Medical (PCM) mental health liaison for external mental health services.
• Facilitate the admission and placement of inmates/patients to forensic hospitals when necessary.
• Monitor all mental health services provided.
• Ensure clinical services meet NCCHC standards and accepted psychiatric/psychological standards.
• Participate as a member of the Quality Assurance Committee.
• Develop and conduct in-service education programs according to the annual in-service calendar.
• Collect data and prepare monthly performance reports.
