About The Position

PrimeCare Medical is seeking a Licensed Professional Counselor (LPC) or Licensed Social Worker (LMSW or LCSW) to serve as a Behavioral Health Professional. This role involves traveling to and working within multiple correctional facilities in Westmoreland, Somerset, Washington, and Cambria Counties in Pennsylvania. The position is full-time, 40 hours per week, with standard business hours from Monday to Friday, 8:00 am to 4:30 pm. The role requires the ability to travel and work across all designated facilities.

Requirements

• Must be licensed as a LPC, LSW, or LCSW in the Commonwealth of Pennsylvania.
• Must hold a Master’s degree or greater in Social Work, Psychology, or related Human Service field with a concentration in clinical coursework, training and/or experience.
• Minimum of two years of experience.

Responsibilities

• Conduct clinical assessments based on referrals and self-referrals, including documentation of findings, mental status examination, case conceptualization, diagnosis, and treatment plan.
• Make referrals to psychiatry when appropriate.
• Conduct risk assessments using structured interviews and risk assessment tools.
• Manage inmates at risk for suicide and/or engaging in self-harm according to established protocols and policies.
• Serve as the PrimeCare Medical (PCM) mental health liaison for external mental health services.
• Facilitate the admission and placement of inmates/patients to forensic hospitals when necessary.
• Monitor all mental health services provided to ensure compliance with NCCHC standards and accepted psychiatric/psychological standards.
• Participate as a member of the Quality Assurance Committee.
• Develop and conduct in-service education programs according to the annual in-service calendar.
• Collect data and prepare monthly performance reports.

• Annual Salary range is $65,000.00 - $75,000.00
