About The Position

Continuum Hospice is seeking a patient-centered Licensed Master Social Worker (LMSW) to join their team. This role focuses on delivering exceptional social work services to hospice patients and their families, providing counseling and guidance to support their emotional well-being and coping capacity during challenging times. The position is per diem, scheduled for two days per week (16 hours), and requires travel within Santa Cruz, and as needed to Monterey and San Benito Counties. Requirements

• Master of Social Work from an accredited school of social work
• Licensed as a Social Worker from the State Board of Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle
• Minimum one (1) year experience as a social worker in a healthcare setting
• CA: Current unencumbered license as a Social Worker from the State Board of Social Work
• CA: Possess and maintains current CPR certification

Nice To Haves

• Minimum one (1) year experience as a social worker in a hospice setting
• Long term care or medical surgical/acute care setting preferred

Responsibilities

• Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
• Evaluate medical needs, home situations, financial resources, and community support.
• Implement personalized social work plans and collaborate with your Bereavement Coordinator.
• Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.
• Provide empathetic counseling to patients and families.
