About The Position

Legend Oaks Healthcare and Rehabilitation Center in Northwest Houston is seeking a Licensed Social Worker for its Skilled Nursing Facilities. This role involves planning, developing, organizing, implementing, and directing social service programs in accordance with federal, state, and local standards, as well as established policies and procedures. The goal is to ensure the medically related emotional and social needs of the resident are met and maintained on an individual basis. The position may involve supervising Social Services Assistants. The role requires staying updated on current regulations and professional standards, and making recommendations for policy changes. It also includes assisting in the development and implementation of policies for identifying resident needs, participating in community planning, and assisting with discharge planning, social care plans, and resident assessments. A compassionate and understanding approach is necessary when interacting with residents and families to gather social history and plan individualized programs.

Requirements

  • Must have, as a minimum, a bachelor’s degree in social work or a bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology.
  • Must be a licensed Social Worker.
  • Must have, as a minimum, 1 year of supervised social work experience in a skilled nursing/ long term care setting working directly with individuals.
  • Must demonstrate exceptional interpersonal and communication skills.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
  • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning.
  • Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing social service.

Responsibilities

  • Plan, develop, organize, implement, and direct social service programs at the facility.
  • Ensure that medically related emotional and social needs of the resident are met/maintained on an individual basis.
  • Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to leadership partners.
  • Review department policies and procedures, at least annually, and participate in making recommended changes.
  • Assist in developing and implementing policies and procedures for the identification of medically related social and emotional needs of the resident.
  • Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
  • Participate in discharge planning, development, and implementation of social care plans and resident assessments.
  • Demonstrate compassion and understanding when working with families and residents during interactions to obtain social history and background information and while planning individualized social service programs.
