About The Position

The Licensed Social Services Director is responsible for maintaining and/or improving residents' ability to control their everyday physical needs and mental/psychosocial needs. This role involves identifying individual social and emotional needs, developing and maintaining individualized care plans, and participating in the planning of resident admissions, community re-entry, or transfers. The director also collaborates with the finance team to ensure proper documentation and resident education regarding financial matters, and works with residents on discharge planning to ensure they have the necessary durable medical equipment (DME) or home care for a safe return to the community. Participation in interdisciplinary care plan meetings is crucial to address the social and emotional needs of residents.

Requirements

BSW or MSW required, or Bachelors degree in related field
At least one year experience in Social Work field required

Nice To Haves

Experience in long term care and/or sub-acute setting preferred

Responsibilities

Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
Identifying individual social and emotional needs
Developing and maintaining individualized care plans
Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
