About The Position

The Licensed Program Coordinator will assess, authorize, supervise, and deliver care to adults with serious mental illness. This position also administers, manages, and operates the Community Support Services (CSS) program by supervising and directing a team of employees. The role involves performing comprehensive rehabilitation needs assessments, contributing to the development, implementation, monitoring, updating, and approval of individualized rehabilitation plans, and performing therapeutic rehabilitative skill development. Services offered are appropriate to meet consumer needs, including crisis intervention and 24-hour on-call support. The coordinator submits care recommendations to the State Interim Management Entity (IME) for authorizations and coordinates, manages, supervises, and delivers all services in conjunction with the CSS team. This includes assessment, evaluation, rehabilitative planning, case conferences, referrals, family meetings, group work, individual counseling, telehealth, consumer advocacy, liaison with community agencies, discharges, and other services defined in CSS regulations. The role ensures accuracy and consistency with fiscal and billing procedures, participates in program development, new initiatives, and marketing/community engagement. Additionally, the coordinator provides support, clinical supervision, and direction to program employees, including orientation, training, performance evaluation, and professional development. They monitor consumer self-administration of medications and assist consumers with accessing community-based wellness and recovery services. Transportation of consumers in Agency vehicles is required occasionally. Documentation of service delivery must be accurate and timely, ensuring consumer confidentiality. The position promotes wellness and recovery practices, consumer choice, and valued life roles.

Requirements

  • NJ Licensed Clinical Social Worker, NJ Licensed Professional Counselor, or NJ Registered Nurse.
  • Knowledge of mental health rehabilitative principles.
  • Valid NJ driver’s license, with a good driving record.

Responsibilities

  • Assessing, authorizing, supervising, and delivering care to adults with serious mental illness.
  • Administering, managing, and operating the Community Support Services (CSS) program by supervising and directing a team of employees.
  • Performing comprehensive rehabilitation needs assessments.
  • Contributing to the development, implementation, monitoring, updating, and approval of the individualized rehabilitation plan.
  • Performing therapeutic rehabilitative skill development.
  • Offering services appropriate to meet the consumers’ needs.
  • Performing crisis intervention and 24 on-call support to employees and consumers for crisis intervention and emergency services linkages.
  • Submitting care recommendations to the State Interim Management Entity (IME) to secure authorizations for care.
  • Coordinating, managing, supervising, and delivering all services and in conjunction with the CSS team including assessment, evaluation, rehabilitative planning, case conferences, referrals for adjunctive services, family meetings, group work, individual counseling, telehealth services, consumer advocacy, liaison with other community agencies, discharges, and any other services outlined or defined in the CSS regulations.
  • Ensuring accuracy and consistency with the agency’s fiscal and billing procedures.
  • Participating in program development, implementation of new initiatives, and marketing/community engagement activities.
  • Providing support, clinical supervision, and direction to all program employees including orientation, training, performance evaluation, and professional development.
  • Monitoring, as needed, consumer self-administration of prescribed medications as per agency procedures.
  • Assisting consumers with accessing and engaging in community-based wellness and recovery services.
  • Transporting consumers, as needed, in Agency vehicles (approx. 2x per yr).
  • Documenting service delivery accurately and timely while ensuring consumer confidentiality.
  • Promoting wellness and recovery practices, a consumer choice focus, and valued life roles for each individual served.
