About The Position

Reports to the Operations Manager, the Licensed Professional Counselor Associate (LPCA) is primarily responsible for providing patient care services, including assessing, diagnosing, treating and educating patients. The LPCA works closely with physicians and other clinicians in a team approach to patient care. The LPCA is a highly visible position that is always responsible for creating a positive impression with patients, physicians and other visitor encounters, both in person and on the phone. The nature of this practice will consist of working with adults who present with substance use disorders and mental illness for treatment. The LPCA will be provided in-house individual and group supervision until the LPCA obtains a clinical licensure of an LPCC.

Requirements

Master's in Clinical Mental Health Counseling degree accredited program
0-2+ years of clinical mental health counseling with substance use disorders.
Demonstrated ability to work successfully in a team-based decision-making culture.
Demonstrated ability to work independently resulting in effective outcomes and on-time performance.
Experience in planning and coordinating multi-disciplinary communications strategies, strategic initiatives, and events.
Must respond and follow through to requests from customers promptly.
Must work carefully and precisely with attention to detail.
Must utilize resources wisely.
Performs duties willingly and with initiative.
Shares necessary information so co-workers can do the same.
Cooperates with other departments and work groups.
Class D driver’s license

Responsibilities

Provides clinical mental health, which focuses on the assessment and collaboration with patient’s treatment providers to determine diagnosis and treatment of a substance use disorder and mental illness as related to the total health of the patient.
The practice of clinical mental health counseling shall be based on knowledge of DSM-5 diagnosis, ASAM, psychodynamics, assessment, human relations and crisis intervention.
Incorporates the six principles of trauma informed care throughout all aspects of their work.
Communicates appropriate and applicable information in chart in accordance with HIPPA guidelines.
Clinical work will be performed under the LPCA Board supervision.
Initiate contact with patient/family in the Physician’s office or by phone within 1 business day from referral date.
Creates a positive therapeutic relationship with the patient.
Provide a wide range of evidence based treatment modalities based on assessment which may consist of: short-term CBT, solution-focused, motivational interviewing; problem solving; crisis intervention; mindfulness; trauma-informed; behavior activation; acceptance and commitment, person-centered or any number of effective evidence-based options to stabilize and regulate the current dysfunction.
Identifies social determinants that impair/impact the patient’s inability to pursue treatment or barriers that inhibit patient’s on-going health issues and works closely with the Outreach Specialist within the Care Coordination department to refer patients for social determinate needs.
Treatment planning that includes development, implementation and modification of the plan.
Evaluation of progress on patients care plans.
Termination of the treatment process or referral to higher level of care when necessary.
Documents all visits and phone contacts accurately and in a timely manner.
Identifies and documents all interventions and case closure plans in EPIC.
Initiates patient/family education, short term counseling and interventions to enhance problem solving and coping.
Acts as a liaison between patient/family and all members of the healthcare team.
Continuously seeks to increase the quality and value of services provided to our patients/family and health care teams
Builds rapport with patients/family and healthcare teams.
Expands and/or strengthens patient’s social supports.
Facilitates support groups as needed to support patient population.
Serves as consultant to the interdisciplinary team regarding psychosocial components, procedures or services to promote greater understanding of the patient.
Communicates effectively with co-workers and healthcare team to ensure continuity of care and service.
Participates in peer reviews, EMR reviews, employee, education, clinical guideline development and other continuing education and quality assurance activities, demonstrate compliance with standards, regulations, policies and procedures.
Complies with patient confidentiality requirements and promotes patient advocacy.
