About The Position

The Licensed Practical Nurse (LPN) provides comprehensive skilled care as ordered by the attending physician by following the order sets created by the RNCM within a specified geographical territory. The LPN educates patients and their family members and ensures the safety of the patient. The LPN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The LPN is responsible for all practices and duties within the scope of practice as outlined by the state. This role involves providing skilled nursing care in a patient’s home, observing and monitoring patient conditions, administering medication, and counseling patients and families on disease processes and management. The LPN will also help decrease re-hospitalizations by teaching preventative measures and assisting patients in making good decisions. They are responsible for completing all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines, maintaining an active LPN license, and communicating any changes to the scheduler. Participation in on-call requirements, case conferences, in-service trainings, and mandatory agency meetings is also expected. The LPN must maintain a professional appearance and adhere to agency policies and procedures.

Requirements

Licensed Practical Nurse with current license in the state of employment.
Minimum of one year of experience as a LPN in an acute care setting is required.
Valid driver’s license and auto insurance with your name as a listed driver.
Must be able to meet all physical position requirements.
Evidence required of a current negative tuberculin skin test or x-ray or receive TB testing at the date of orientation.
Thereafter an annual tuberculin skin test is required, unless the employee has documentation of a previous TB test.
Employee must be free of physical/medical conditions, which would limit or restrict their ability to perform the job functions listed below.

Nice To Haves

Two years of experience as a LPN in an acute care setting is preferred.
Home care experience is desired.

Responsibilities

Provides skilled nursing care in a patient’s home as ordered by the attending physician.
Performs care by following order sets and goals made by RN.
Observes and monitors patient conditions.
Notifies RNCM of any changes in patient condition.
Administers medication as prescribed by the physician.
Counsels patient and family on the disease/injury processes and how to manage.
Incorporates patient and family in development of plan of care.
Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
Listens to patient and family members.
Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
Documents all aspects of treatment, assessments, and patient education.
Maintains active LPN license in state of employment.
Communicates with scheduler any changes outside the normal.
Participates in all on-call requirements and case conferences.
Attends in-service trainings and mandatory agency meetings.
Maintains a professional appearance as a representative of the company.
Understands and practices agency policies and procedures.
Drives to patient’s primary location per scheduled visit.
Daily attendance at assigned visit locations.
Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines.
Completes and submits all required documentation within specified company requirements.
Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.
