About The Position

Caringhand Home Health Solutions, Inc. provides professional and paraprofessional home-based care intended to help clients achieve the highest level of potential in their day-to-day self-care activities. The organization focuses on multidisciplinary care and comprehensive assessment, and supports patients transitioning home from medical facilities. The Licensed Practical/Vocational Nurse provides field-based care in patients’ homes, helping them recover safely and effectively. This is a per diem role requiring travel to patient homes; despite being listed as remote, it is not a work-from-home or telehealth position. Compensation is $45–$55 per visit. The position may convert to full-time based on performance if the employee is interested.

Requirements

Graduate of an accredited practical or vocational nursing program.
Active LPN or LVN license in California.
3+ years of nursing experience.
Current CPR certification with AHA.
Valid driver’s license, reliable insured vehicle, and ability to travel to patient homes.
Strong communication skills and a collaborative, patient-first mindset.

Nice To Haves

Home health or med/surg background preferred.

Responsibilities

Provide direct patient care in accordance with the state Nurse Practice Act.
Implement and follow individualized care plans established by the RN.
Educate patients and their families on self-care techniques and diagnosis-related information.
Document care accurately and in a timely manner.
Collaborate with the care team, including OT, PT, HHA, and other disciplines, and communicate patient needs to the RN or Clinical Supervisor.
Use supplies and equipment safely, adhering to aseptic technique.
Perform other duties as required for patient services.
