About The Position

It also supports patients transitioning home from medical facilities with expert guidance during recovery.

As a Licensed Practical/Vocational Nurse, you will provide direct care to patients in their homes, help them heal and remain safe, and support patients and families during recovery. This is a field-based, per diem position requiring travel to patient homes. Although listed as remote, it is not a work-from-home or telehealth role. Compensation is $45–$55 per visit.

Requirements

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• At least 3 years of nursing experience.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Nice To Haves

• Home health or med/surg background preferred.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team, including OT, PT, HHA, and other disciplines, and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
