The Licensed Practical Nurse (LPN) plays a crucial role in supporting providers in delivering high-quality primary care. This role involves direct and indirect patient care, adhering to state scope of practice regulations and working under the guidance of physicians, advanced practice providers, or registered nurses. Key responsibilities include performing clinical duties, educating patients, administering medications, collecting specimens, coordinating care, and contributing to efficient clinical workflows and an excellent patient experience.

  • Graduate of an accredited Practical Nursing Program.
  • Active, unrestricted Florida Licensed Practical Nurse (LPN) License required.
  • Current Basic Life Support/AED Certification from an AHA-accredited provider.
  • Must maintain all continuing education requirements required by Florida Board of Nursing.
  • Child Clearance - Act 33.
  • Criminal Clearance - Act 38.
  • FBI Clearance - Act 73.

Nice To Haves

  • 2–5 years of outpatient, ambulatory, or primary care nursing experience preferred.
  • Experience in value-based care, population health, or preventive medicine preferred.

Responsibilities

  • Provides direct and indirect patient care under the direction of a licensed physician, advanced practice provider, or registered nurse, consistent with state scope of practice regulations.
  • Supports providers in delivering high-quality primary care by performing clinical duties, patient education, medication administration, specimen collection, care coordination, and operational support.
  • Ensures efficient clinical workflows and an exceptional patient experience.
  • Maintains a working knowledge of PeopleOne Health membership plans and how to access needed information.
  • Guides the member experience by providing information, education regarding options for care, and available resources.
  • Manages, conducts, and tracks outreach initiatives for the member population.
  • Provides exceptional customer service by answering inquiries regarding services, claims, benefits, eligibility, provider searches, and other health-related questions, and following up with members.
  • Utilizes technology, data, and internal information to assist employees in obtaining the best and most cost-efficient health care for disease and condition management.
  • Manages office telephone systems, including answering calls and returning messages.
  • Schedules patient appointments and maintains communication to confirm future appointments.
  • Scans and enters Protected Health Information and documents into the patient’s chart.
  • Performs data entry tasks to document patient records within facility databases and maintains patient files.
  • Ensures patients are active members to receive services via the member portal.
  • Ensures patients are pre-approved for care by collecting prior authorization from the insurance provider for outgoing referrals, when necessary.
  • Coordinates daily office activities, greets patients, and checks patients in and out for appointments.
  • Performs patient intake, including recording vital signs, medical history updates, medication reconciliation, and chief complaint documentation.
  • Assists providers with patient examinations, procedures, and treatments.
  • Administers medications and immunizations via approved routes under provider order and within state scope of practice.
  • Performs venipuncture, specimen collection, specimen processing, and prepares laboratory samples for transport/analysis.
  • Performs point-of-care and CLIA-waived testing per policy and competency validation.
  • Conducts wound care, dressing changes, suture/staple removal, ear irrigation, EKGs, nebulizer treatments, and other delegated nursing tasks within scope.
  • Performs patient triage and symptom assessment, escalating clinical concerns appropriately.
  • Prepares and stocks exam rooms and procedural areas.
  • Maintains infection prevention and control standards, including proper biohazard handling.
  • Assists with medication inventory, vaccine management, and clinical supply monitoring.
  • Completes standing order and protocol-based care activities as directed.
  • Coordinates referrals, diagnostic testing, and follow-up care as directed.
  • Assists with care gap closure and quality measure outreach.
  • Supports chronic disease management and preventive care outreach initiatives.
  • Accurately documents patient care, interventions, and communications in the electronic medical record.
  • Processes telephone encounters, refill requests, prior authorizations, and patient messages per workflow.
  • Maintains confidentiality and HIPAA compliance in all patient interactions.
  • Participates in chart preparation and pre-visit planning.
  • Assists with clinical audits, compliance monitoring, and quality initiatives.
  • Maintains active awareness of the State LPN scope of practice and practices only within authorized limits.
  • Utilizes daily/weekly/monthly checklists to ensure clinical, regulatory, and safety compliance.
  • Participates in emergency preparedness drills and clinical competency validations.
  • Supports infection control, OSHA, CLIA, and regulatory readiness activities.
  • Performs additional responsibilities as assigned.
