Requirements

  • Completion of a state-approved Practical Nursing program (typically 12-18 months).
  • Passing the NCLEX-PN to obtain licensure.
  • State licensure as a Licensed Practical Nurse (LPN) with renewal as required.
  • CPR/BLS certification often mandatory prior to employment.
  • Understanding the specific needs of elderly patients, especially those with dementia or chronic ailments.
  • Adept in care planning, medication administration, and documentation compliance.
  • Clinical proficiency must include wound care and infection control protocols.
  • Strong communication skills.
  • Ability to manage time and prioritize tasks in a dynamic environment.
  • Familiarity with federal and state regulations (e.g. CMS OBRA) governing SNF operations and patient rights.
  • Strict adherence to infection prevention protocols.

  • Additional training in geriatric care.
  • Additional geriatric care certifications.

Responsibilities

  • Receiving patient reports and reviewing care plans.
  • Monitoring vital signs and administering medications.
  • Changing dressings and assisting with personal hygiene.
  • Collaborating with Registered Nurses (RNs) and physicians to communicate essential updates on patient conditions.
  • Providing education and emotional support to residents and their families.
  • Assisting in the development, implementation, and monitoring of individualized care plans.
  • Administering routine medications, observing effects, and ensuring proper documentation and compliance.
  • Supporting mobility, independence, and rehabilitation goals of residents.
  • Delivering compassionate, respectful care tailored to each resident's needs.
  • Performing wound care, catheter care, and tube feedings.
  • Monitoring for changes in resident conditions.
  • Accurate and timely charting in accordance with SNF policies and legal standards.
  • Handling multiple residents' needs in a fast-paced, routine-driven environment.
  • Working under the supervision of RNs and coordinating with CNAs and other healthcare staff.
