The LPN provides direct primary nursing care to residents and supervises the care provided by certified nursing assistants. This role involves keeping administrative nursing staff informed on personnel development, providing feedback to Nurse Managers regarding the performance of nursing assistants, and participating in committees. The LPN demonstrates the ability to address service concerns and follow up timely for resolution, supporting the facility’s Service Excellence Initiative. This position effectively supervises and directs licensed and non-licensed nursing personnel, and is responsible for developing CNA resident care assignments and other duties as needed prior to the beginning of each shift. The LPN promotes and maintains positive and collaborative working relationships with physicians, physician-extenders, lab, diagnostic personnel, pharmacy, nursing personnel, other departments, residents, families, and visitors. Key responsibilities include observing residents daily, reporting significant changes in condition to the physician, completing routine nursing documentation accurately and timely, and administering medications using the five rights of medication administration while adhering to infection prevention and control practices. The LPN also provides and receives shift reports, implements pharmacy processes, performs physical assessments, and recognizes normal and abnormal physiological findings. Wound care is performed in accordance with physician orders, and the LPN assists in the development and updating of the resident’s plan of care. Communication with family members regarding resident care and/or condition is essential. The LPN completes accident/incident reports, notifies the physician and family of incidents, and performs various nursing skills such as phlebotomy, IV therapy, wound care, medication administration, catheter insertion, tube feeding management, oxygen administration, tracheostomy care, airway management, and physical/skin assessments. The LPN communicates staffing problems and needs, reports service and unit concerns, and receives/provides end-of-shift reports. Responsibilities also include assisting with resident admissions, transfers, and discharges, reporting maintenance and housekeeping needs, and accurately maintaining narcotic records, including validation before and after shifts and notifying management of discrepancies.
