About The Position

Homestead Village, a non-profit, CARF-CCAC accredited Life Plan Community, is seeking a skilled, enthusiastic, and passionate Licensed Practical Nurse (LPN) to join their compassionate community. The role focuses on enhancing independence and dignity for residents within the Main Apartment Building's Supportive Services. This is a full-time, non-exempt position with a schedule of 6:45 PM - 7:15 AM, involving three 12-hour shifts per week, including every third weekend and every other holiday. The pay is determined by licensed years and experience, with a starting rate of $27.00 for LPNs with less than one year of experience, plus applicable shift and weekend differentials.

Requirements

Currently licensed in the Commonwealth of PA as a Licensed Practical Nurse
Minimum of three years as an LPN
Minimum of three years of supervisory experience
Experienced in developing support plans for elders
Familiarity with Personal Care Home Regulations
Hands-on experience in ADL assistance
CPR/AED certified or able to be certified
Written & verbal skills to communicate with client, families, and staff.
Able to perform the essential job functions of the job description.

Responsibilities

Establish respectful, warm relationship with residents, perform rounds
Admission of residents
Develop/update of resident pre-screen, assessment, and support plans
Communicate, as necessary, with physicians and resident families.
Staff supervision and coaching
Assist with meeting resident activities of daily living as needed/indicated.
Assist with medication administration as needed
Participate in weekly PCH Supervisory meetings
Assist with emergency response campus-wide, as needed
Attend all required staff meetings and complete monthly education on Relias by dates due.
Adhere to all agency policies and procedures, federal, state, and local regulations, and OSHA safety standards.
Perform other duties as assigned
