About The Position

Concordia Visiting Nurses is looking for a dedicated and empathetic LPN to join our growing home health team! As a Home Health LPN, you will be a vital part of the patient's care team, offering skilled nursing care and support. This includes monitoring vital signs, assisting with wound care and personal care, educating patients and families, maintaining medical records, and collaborating with other healthcare professionals. You will also provide emotional support and companionship, building genuine connections with patients.

Requirements

• A minimum of one year of LPN experience
• Excellent communication, interpersonal, and organizational skills
• Ability to work independently and as part of a collaborative team
• Compassion, empathy, and emotional resilience to provide comfort and support during end-of-life situations

Nice To Haves

• Hospice experience is preferred but not required
