About The Position

Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Our goal is to deliver quality home health services, in good faith, to those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity, and compassion. We strive to hire excellent people at Concordia - and keep them as well! Concordia Visiting Nurses is looking for a dedicated and empathetic LPN to join our growing home health team!

Requirements

• A minimum of one year of LPN experience
• Excellent communication, interpersonal, and organizational skills
• Ability to work independently and as part of a collaborative team
• Compassion, empathy, and emotional resilience to provide comfort and support during end-of-life situations

Nice To Haves

• Hospice experience is preferred but not required

Responsibilities

• Monitoring vital signs
• Assisting with wound care and personal care
• Educating patients and families
• Maintaining medical records
• Collaborating with other healthcare professionals
• Provide emotional support and companionship, building genuine connections with patients
