About The Position

The Licensed Practical Nurse (LPN) plays a critical role in delivering high-quality nursing care to residents within a nursing home. This position focuses on providing direct patient care, managing wound care, and supporting pain management protocols to enhance resident comfort and recovery. The LPN will collaborate closely with the Nurse Manager or Supervisor and other healthcare professionals to implement individualized care plans. A key outcome of this role is to maintain resident safety and promote health through skilled nursing interventions and compassionate support. Provide oversight to CNA staff assigned to the unit. Ultimately, the LPN contributes to the overall well-being and improved health outcomes of residents by applying clinical knowledge and practical nursing skills in a professional and empathetic manner.

Requirements

  • Valid and current Nursing License as a Licensed Practical Nurse (LPN) in the United States, specifically a valid NYS license to practice.
  • Proficiency in reading, writing, and speaking English to effectively communicate with patients and healthcare team members.
  • Demonstrated experience in wound care and patient care within a clinical or assisted living setting.
  • Ability to perform venipuncture and other basic nursing procedures safely and competently.
  • Strong understanding of pain management principles and patient education techniques.
  • Strong communication skills.

Nice To Haves

  • Experience working in an assisted living facility or similar long-term care environment.
  • Additional certifications related to wound care or pain management.
  • Familiarity with electronic health record (EHR) systems and healthcare documentation software.
  • Continuing education or specialized training in geriatric nursing or chronic disease management.

Responsibilities

  • Provide direct patient care including monitoring vital signs, administering medications, and performing wound care treatments.
  • Provide oversight of all CNA's assigned to the unit.
  • Educate patients and their families on health management, medication usage, and post-care instructions to promote recovery and prevent complications.
  • Perform venipuncture and other routine clinical procedures as required to support diagnostic and treatment processes.
  • Collaborate with Nurse Practitioners and interdisciplinary teams to develop and implement individualized care plans.
  • Document patient information accurately and maintain compliance with healthcare regulations and nursing standards.
  • Assist in pain management by assessing patient discomfort and administering prescribed interventions.
  • Support the daily operations of assisted living facilities by ensuring a safe and therapeutic environment for residents.
