About The Position
Lutheran Homes & Health Services, an Illuminus Community, provides assisted living, skilled nursing, and short-term rehabilitation services to older adults in Fond du Lac, Wisconsin. The Licensed Practical Nurse (LPN) identifies and provides for residents’ nursing needs and accepts responsibility for their care. Under the general supervision of a registered nurse, the LPN provides prescribed medical treatment and performs LPN practice activities up to and including medication administration, monitoring, and documentation while ensuring person-centered care. The role may include 8-hour shifts and weekend rotation as scheduled; full-time, part-time, and PRN opportunities and varied scheduling options are available, subject to community needs.
Requirements
- Current Licensed Practical Nurse credentials and a license in the State of Wisconsin.
- Active CPR.

- Ability to exercise independent judgment and make sound decisions.

Responsibilities
- Identify, provide for, and accept responsibility for residents’ nursing needs.
- Develop and implement care plans; administer medications and treatments; monitor resident health; assist with physician visits; and verify physician orders.
- Lead and supervise CNAs and resident assistants, promote quality resident-centered care, and follow safety, infection-control, and PPE requirements.
- Observe, document, and report residents’ status and changes in condition; complete required documentation; and communicate concerns and care information to the appropriate nurse, family member, or physician.
- Maintain professional communication with residents, families, and staff while promoting the organization’s mission, values, and positive community image.
