About The Position

Anchor Post Acute, a 120-bed skilled nursing & rehab facility in Aiken, South Carolina, is seeking compassionate Licensed Practical Nurses (LPNs) to join their award-winning team. The facility emphasizes building strong relationships with residents, families, coworkers, and the community, and is looking for individuals passionate about providing exceptional care. The role involves providing direct nursing care to residents and supervising Certified Nursing Assistants (CNAs) to ensure the highest quality of care while adhering to all federal, state, and facility guidelines.

Requirements

Nursing Degree from an Accredited College or University
ACTIVE, Unencumbered South Carolina LPN License
Ability to Work 12-Hour Shifts
Ability to Work Weekends
Must be able to speak and write the English language in an understandable manner.
Must be able to see and hear or use prosthetics that will enable these senses to function adequately.
Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
Current CPR certification.
Must maintain all required continuing education/licensing.
Must remain in good standing with the State Board of Nursing at all times.

Nice To Haves

Experience in a Fast-Paced, High-Admissions Healthcare Setting Preferred
Knowledge and experience with PCC preferred.

Responsibilities

Provide direct nursing care to residents.
Supervise the day-to-day nursing activities performed by Certified Nursing Assistants.
Ensure compliance with federal, state, and facility guidelines.
Administer medications and treatments as ordered by physicians.
Complete and file required recordkeeping forms/charts.
Transcribe physician's orders.
Chart nurses' notes in an informative and descriptive manner.
Participate in the orientation of new residents and families.
Consult with resident's physician regarding care and treatment.
Implement and maintain established nursing objectives and standards.
Perform restorative and rehabilitative programs.
Participate in in-service training programs.
Ensure safety and sanitation standards are met.
Participate in the infection control program.
Assist in the development and implementation of procedures for safe operation of nursing equipment.
Review care plans daily and inform Nurse Supervisor of necessary changes.
Assist with resident assessments and care plan coordination.
Participate in the development, planning, and conducting of in-service training classes.
Maintain a liaison with residents, families, and support departments.
Admit, transfer, and discharge residents as necessary.
Call funeral homes when requested by the family and ensure post-mortem procedures are followed.
