Behavioral Health Nurse LPN- Afternoon/Evenings
About The Position
Innovive Health is seeking a Licensed Practical Nurse (LPN) for its Home Health team. The LPN provides high-quality nursing care to patients in their homes under the supervision of a Registered Nurse and in collaboration with an interdisciplinary care team. The role supports patients in managing their health, maintaining independence, and improving their quality of life. This position offers independent, flexible work in a supportive team environment, with one-on-one patient care and a predictable Monday-through-Friday schedule. Available shifts are 7:00 AM–3:00 PM or 12:00 PM–8:00 PM. There are no overnight, weekend, or holiday shifts.
Requirements
- Graduate of an accredited Practical Nursing program.
- Current, unrestricted Massachusetts LPN license.
- Successful completion of NCLEX-PN.
- Current CPR certification.
- Strong clinical assessment, communication, and documentation skills.
- Valid driver's license, reliable transportation, and automobile insurance.
- Ability to meet required health screening and clearance requirements.
- Professional references demonstrating clinical competence and reliability.
- Compassionate, patient-centered approach to care.
- Excellent communication and interpersonal skills.
- Strong organizational and time-management abilities.
- Ability to work independently while remaining connected to a supportive care team.
- Professionalism, reliability, and attention to detail.
Nice To Haves
- Associate degree.
- Two or more years of nursing experience.
- Experience in home health, hospice, long-term care, community health, or related settings.
Responsibilities
- Provide skilled nursing care according to individualized care plans.
- Administer medications and treatments safely and accurately.
- Monitor patient conditions and report changes to the supervising RN.
- Collaborate with physicians, case managers, caregivers, and other healthcare team members.
- Educate patients and families on disease management, medications, and treatment plans.
- Maintain accurate and timely clinical documentation.
- Support patients with dignity, compassion, and respect.
- Participate in care conferences, team meetings, and ongoing training.
