About The Position

Innovive Health is seeking a Licensed Practical Nurse (LPN) to provide home health nursing care to patients in their homes. Under the supervision of a Registered Nurse and in collaboration with an interdisciplinary care team, the LPN helps patients manage their health, maintain independence, and improve their quality of life. The role offers independent, patient-centered work, meaningful one-on-one care, and a supportive team environment. Available Monday-Friday shifts are 7:00 AM–3:00 PM or 12:00 PM–8:00 PM, with no overnights, weekends, or holidays.

Requirements

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted Massachusetts LPN license.
• Successful completion of NCLEX-PN.
• Current CPR certification.
• Strong clinical assessment, communication, and documentation skills.
• Valid driver's license, reliable transportation, and automobile insurance.
• Ability to meet required health screening and clearance requirements.
• Professional references demonstrating clinical competence and reliability.
• Compassionate, patient-centered approach to care.
• Excellent communication and interpersonal skills.
• Strong organizational and time-management abilities.
• Ability to work independently while remaining connected to a supportive care team.
• Professionalism, reliability, and attention to detail.

Nice To Haves

• Associate degree.
• Two or more years of nursing experience.
• Experience in home health, hospice, long-term care, community health, or related settings.

Responsibilities

• Provide skilled nursing care according to individualized care plans.
• Administer medications and treatments safely and accurately.
• Monitor patient conditions and report changes to the supervising RN.
• Collaborate with physicians, case managers, caregivers, and other healthcare team members.
• Educate patients and families on disease management, medications, and treatment plans.
• Maintain accurate and timely clinical documentation.
• Support patients with dignity, compassion, and respect.
• Participate in care conferences, team meetings, and ongoing training.
