Requirements
- Current, active Licensed Practical Nurse (LPN) license in the applicable state.
- Graduate of an accredited Practical Nursing program.
- Strong clinical judgment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to lead and mentor Certified Nursing Assistants while fostering a collaborative team environment.
- Commitment to compassionate, resident-centered care.
- Ability to read, write, speak, and understand the English language at an intermediate or more advanced level.
Nice To Haves
- Long-term care, rehabilitation, or skilled nursing experience is preferred but not required.
Responsibilities
- Administer medications and treatments according to physician orders and established nursing standards.
- Monitor residents for changes in condition and promptly communicate concerns to physicians, Registered Nurses, and interdisciplinary team members.
- Supervise and support Certified Nursing Assistants while promoting teamwork and quality resident care.
- Assist with direct resident care as needed.
- Implement and monitor individualized care plans to ensure residents receive person-centered care.
- Complete accurate and timely clinical documentation, including medication administration records, treatment records, nursing notes, and physician orders.
- Safely manage controlled medications in accordance with facility policies and regulatory requirements.
- Participate in physician rounds, care conferences, admissions, transfers, and discharge planning.
- Maintain compliance with infection prevention practices, quality assurance initiatives, and state and federal regulations.
- Collaborate with rehabilitation, dietary, social services, and nursing teams to promote positive resident outcomes.
