About The Position

Vanderbilt Health is seeking a Licensed Practical Nurse (LPN) to provide direct patient care in a home setting for both pediatric and adult patients. This role operates Monday through Friday, with the possibility of 12-hour shifts that may include some weekends. The position offers the opportunity to build meaningful one-on-one relationships with patients and families, gain exposure to a wide range of diagnoses and age groups, and develop advanced clinical expertise. LPNs will collaborate with RNs and other healthcare providers, follow established care plans, and support health promotion initiatives. They will navigate intense medical scenarios under the direction of a supervising RN, administer IV fluids, perform sterile procedures, and respond to clinical situations appropriately. Accurate and timely reporting of patient needs and clinical changes is essential.

Requirements

  • LIC-Licensed Practical Nurse - Tennessee
  • Relevant Work Experience
  • Less than 1 year experience
  • Graduate of an approved discipline specific program

Responsibilities

  • Collaborates with patients/families and health care providers in the delivery of health care services.
  • Partners with the healthcare team by following established plan of care to ensure patient progress for effective outcomes.
  • Support plans to promote health and a safe and healthy environment.
  • Navigates intense medical scenarios with autonomy, under the direction of the supervising RN while maintaining compliance with the physician ordered plan of care.
  • Administer intravenous fluids and perform sterile procedures.
  • Recognize and respond to clinical situations appropriately and timely to prevent patient decompensation and injury.
  • Identify and assess patient needs and clinical changes, accurately reporting assessment findings in a timely manner to Supervising RN and physician.
