About The Position

The Licensed Practical Nurse supports the Quality and Data Analytics team. The role prepares AWV-related orders ahead of patient visits, conducts patient outreach calls, reviews provider documentation for completeness, and coordinates patient scheduling with practices. The LPN works with TMG practice providers, managers, and directors to ensure patient care gaps are addressed in a timely manner.

Requirements

• Associate degree or two years of college coursework.
• One year of related experience.
• Working knowledge of standard practices and procedures.
• Completion of a formal education program in practical nursing.
• Current Alabama LPN licensure or a multistate nursing license.

Nice To Haves

• Prior outpatient, quality, or population health experience.
• Experience with EMRs, data abstraction, patient outreach, or value-based care initiatives.
