About The Position

This LPN supports the Quality and Data Analytics team. The role prepares and enters Annual Wellness Visit (AWV)-related orders before patient visits, conducts patient outreach calls, reviews provider documentation for completeness, and coordinates with practices to schedule patients. The LPN works with TMG practice providers, managers, and directors to ensure patient care gaps are addressed in a timely manner.

Requirements

• Associate degree or two years of college coursework.
• One year of related experience.
• Working knowledge of standard practices and procedures.
• Completion of a formal education program in practical nursing.
• Current state LPN licensure.
• Alabama Licensed Practical Nurse license or multistate nursing license.

Nice To Haves

• Prior outpatient, quality, or population health experience.
• Experience with EMRs, data abstraction, patient outreach, or value-based care initiatives.
