About The Position
The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role involves supporting patients and caregivers through education, monitoring, medication review, and ordered procedures while coordinating with the RN and physician. The position includes both per-visit-point compensation for visits and hourly pay for certain non-visit activities, such as orientation.
Requirements
- Current LPN license specific to the state(s) assigned for work.
- Valid driver's license, reliable transportation, and liability insurance.
- Current CPR certification.
- At least 1 year of LPN or LVN experience.
Responsibilities
- Verify the plan of care with the RN prior to each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are being met, and notify them as soon as possible when a patient's condition changes.
- Teach patients and caregivers self-care techniques.
- Check vital signs.
- Review all medications and perform medication reconciliation at each visit.
- Perform specialized procedures ordered by a physician, such as Foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation accurately and on time.
