About The Position
The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role supports patients through personalized care and helps build trusted relationships with patients and caregivers. The position is compensated through per-visit-point pay for visits and hourly pay for certain non-visit activities, such as orientation. Annual total cash compensation assumes full-time employment at 40 hours per week and full productivity; actual compensation may vary based on factors including local labor markets, education, work experience, productivity, and performance. Visits are assigned according to patient and business needs.
Requirements
- Current LPN license specific to the state(s) assigned for work.
- Current CPR certification.
- At least 1 year of LPN or LVN experience.
- Valid driver's license, reliable transportation, and liability insurance.
- Must pass a drug test before beginning employment.
Responsibilities
- Verify the plan of care with the RN before each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
- Teach patients and caregivers self-care techniques.
- Check vital signs.
- Review medications and perform medication reconciliation at each visit.
- Perform specialized procedures ordered by the physician, such as Foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation accurately and promptly.
