About The Position

PrimeCare Medical, Inc. is seeking dedicated and hard-working Licensed Practical Nurses (LPN) to work in the medical department at the Kent County Detention Center, located in Chestertown, MD. This full-time evening position involves providing comprehensive nursing care and performing duties under the supervision of an RN, with a focus on the Medication Assisted Treatment (MAT) Program. The role requires managing electronic medical records, ensuring security, and coordinating care for patients receiving medications for substance use disorder. A 2-week orientation period with on-the-job training and shadowing is provided.

Requirements

  • Graduate of an accredited School of Practical Nursing.

Responsibilities

  • Deliver nursing care for Medication Assisted Treatment Program in accordance with PCM Policies and Procedures, ensuring compliance with established nursing standards and scope of practice limitations.
  • Function as MAT Nurse and coordinate care for all patients receiving medications for substance use disorder.
  • Provide comprehensive nursing care while observing and maintaining security requirements.
  • Perform duties under supervision of an RN pursuant to protocols established by the Medical Director.
  • Work collaboratively with a multidisciplinary team of PCM nurses, providers, security, administration, and community providers.
  • Coordinate and implement daily dosing of MAT medications in accordance with facility procedures and PCM dosing policy.
  • Maintain a professional working relationship with facility staff, unit staff, providers, and corporate staff.
  • Act as MAT patient liaison through permitted communications on behalf of PCM, the contracted facility, and outside agencies.
  • Coordinate care for all MAT Patients and/or applicable candidates by obtaining records, coordinating care, and making appropriate referrals.
  • Ensure compliance with established standards of PCM and follow established Corporate and facility policies and procedures for MAT in clinical practice.
  • Ensure the proper flow of relevant information (inmate/patient information, unit problems) is disseminated to appropriate personnel.
  • Maintain security and privacy of all medical records, documenting and communicating in accordance with HIPPA and 42 CFR guidelines for confidentiality.
  • Monitor nursing care to avoid the inappropriate use and/or duplication of services, advising HSA and charge nurse if these situations arise.
  • Screen for appropriate referrals to providers for MAT and ensure all follow-up tasks are created.
  • Ensure strict compliance with all applicable pharmacy regulations for ordering, storing, and administration of Medications for Opioid Use Disorder and Alcohol Use Disorder.
  • Monitor stock inventories of supplies and equipment and alert the head nurse of needs.
  • Obtain regular lab specimens as required by MAT program policy.
  • Assist patients with community reentry by linking to community MAT providers, obtaining ROIs, and faxing all pertinent records.
  • Attend scheduled Interdisciplinary Team Meetings for MAT weekly and keep recorded minutes.
  • Report data as required and participate in program evaluation, maintaining ongoing records pertaining to MAT patients for quarterly and yearly statistics.
  • Attend seminars, workshops, and conferences at the direction of PCM.
  • Be flexible in work schedule, as unpredictable circumstances may warrant additional work hours and overnight travel to facilities.
  • May be required to relocate (temporarily) to meet the needs of PCM.
  • Adhere to established PCM and facility policies and procedures.
  • In the absence of the charge nurse, be operationally responsible and accountable for the nursing functions of the medical unit.
  • Provide other services and perform other duties as assigned by charge nurse, HSA, or Corporate staff.
  • Maintain accurate records of all controlled substances, narcotics, and sharps in accordance with applicable state and federal laws.
  • If necessary, conduct sick call (if permitted by state regulations) for inmate/patients and provide treatment for selected minor illnesses and injuries, defined by written protocol and approved by PCM Medical Director. Refer all cases not covered by written protocol or that do not respond to treatment to an appropriate PCM physician.
  • Receive and give inter-shift report to ensure continuity of care and to disseminate pertinent information necessary to the functioning of the oncoming shift.
  • Observe, report, and document symptoms, reactions, and progress of inmates/patients and participate in the formulation of the plan of MAT care.
