About The Position

Amedisys is hiring a full-time Licensed Practical Nurse for home health and hospice care. The role supports patients receiving care at home and involves coordinating with registered nurses and physicians, providing patient and caregiver education, monitoring patients, and documenting care. The position offers a base rate of $31–$36, converted to per-visit compensation, and includes mileage reimbursement. Requirements

One year of clinical LPN experience.
Current LPN license specific to the state or states where assigned to work.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Verify the plan of care with the RN before each visit.
Collaborate frequently with physicians and the RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
Teach patients and caregivers self-care techniques.
Check vital signs.
Review medications and perform medication reconciliation at each visit.
Perform specialized procedures ordered by a physician, such as Foley catheter changes.
Prepare treatment equipment and materials using aseptic technique.
Complete documentation accurately and on time.
