About The Position

Benchmark is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care to residents in a warm, comfortable, home-like environment. The LPN works with a team of dedicated associates and supports residents while balancing service needs with resident independence. The role includes assessing and documenting resident status, communicating changes in health or behavior, supervising and mentoring care staff, coordinating care with community providers, and responding to emergencies.

Requirements

• Current state LPN license and CPR certification.
• Graduate of an approved LPN program, in accordance with state requirements.

Nice To Haves

• One to two years of experience working as a nurse is preferred.
• Previous experience working with elderly people in a paid or volunteer position is highly desirable.

Responsibilities

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in resident behavior and health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director in accordance with state regulations.
• Direct and supervise the daily work assignments of Resident Care Assistants.
• Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their work.
• Coordinate care needs with community providers through an effective case management process.
• Balance service demands with support for resident independence.
• Recognize and help prevent elder abuse, neglect, and exploitation, and report concerns to appropriate sources in accordance with state regulations.
• Respond to emergencies and personal emergency response system alerts, and know when to call for backup.
