Licensed Practical Nurse (LPN) – Home Health Visits – Full-Time. Growing in Blairsville, Georgia and surrounding area. Offering Pay-per-Visit Rates - $35.00 per point - $27.00 base rate. Flexible scheduling, comprehensive benefits package and benefit eligibility upon start date. Mountain Home Health Services provides quality home health care to thousands of individuals in Georgia. We are growing in the Blairsville, Georgia and surrounding county area, and looking to add a Licensed Practical Nurse to our home-health, clinical field team. Requirements

Graduate of an accredited Licensed Practical Nurse educational program
Current Licensed Practical Nurse (LPN) license within the state of practice
Current CPR certification
Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
Strong initiative, follow through, process orientation, attention-to-detail
Reliable transportation, valid driver’s license and current automobile insurance

Responsibilities

Conducting one-on-one patient care and family teaching in the home setting.
Administering skilled nursing care to patients in their place of residence as prescribed and in accordance with the established plan of care.
Timely documentation and collaboration with the interdisciplinary team.
