About The Position

As a Home Health LPN at Trinity Health At Home, you’ll provide skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse. Trinity Health At Home – Grand Rapids is a member of Trinity Health At Home, a national home care, hospice and palliative care organization serving communities throughout eleven states. Requirements

• Graduate of an accredited or state approved school of practical nursing
• Licensure as a LPN in the State of Michigan
• A minimum of one (1) years of experience in an acute care setting
• Phlebotomy and wound care experience required
• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications
• Must possess interpersonal skills sufficient to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors
• Must have current Driver’s license and reliable transportation to and from work site
