About The Position

The Licensed Practical Nurse (LPN) provides direct patient care and clinical support to the physician and care team in a concierge primary care practice. This role plays a vital part in delivering a personalized, high-touch patient experience by assisting with clinical care, coordinating follow-up services, supporting preventive health initiatives, and fostering strong patient relationships. The LPN serves as a trusted resource for patients, helping ensure continuity of care, exceptional service, and adherence to clinical standards and practice protocols. This practice goes beyond traditional insurance-based medicine by offering advanced, cutting-edge testing and personalized care that most patients never experience. Requirements

  • Graduate of an accredited Practical Nursing program.
  • Current Licensed Practical Nurse (LPN) license in the state of practice.
  • Current American Heart Association BLS Certification.
  • Phlebotomy experience or demonstrated proficiency in blood collection.
  • Minimum two years of clinical nursing experience, preferably in primary care, family medicine, concierge medicine, or an outpatient setting.
  • Strong clinical, communication, and patient service skills.

Nice To Haves

  • Experience in a clinical setting, particularly in outpatient practices.
  • Strong clinical assessment and triage skills.
  • Familiarity with specific EMR systems (e.g., EPIC, Athena).
  • Experience with patient education and health promotion.

Responsibilities

  • Build meaningful relationships with a select patient panel.
  • Work closely with our physician in a true collaborative partnership during patient examinations and procedures, ensuring a comfortable and efficient environment.
  • Implementation and management of Chronic Care Management (CCM), including accurate documentation, patient enrollment, monthly call compliance, and revenue contribution
  • Provide direct patient care including vital signs, medication administration, injections, immunizations, wound care, and diagnostic testing within scope of practice.
  • Manage patient records by accurately inputting information into electronic health records (EHR) and maintaining confidentiality in accordance with HIPAA regulations.
  • Coordinate comprehensive care plans and follow up with patients personally.
  • Serve as a primary point of contact, responding to patient needs with care and attention.
  • Apply expanded clinical skills in a thoughtful, patient-centered environment.
  • Maintain accuracy in patient records and ensure compliance with medical protocols.
  • Effectively interact with patients and medical staff, fostering a welcoming and informative atmosphere.
  • Work cohesively with healthcare teams to deliver comprehensive patient care.
  • Respond to changing clinical demands and patient needs in a fast-paced environment.
  • Demonstrate empathy and understanding while addressing patient concerns and inquiries.
