The Licensed Practical Nurse (LPN) provides direct patient care and clinical support to the physician and care team in a concierge primary care practice. Requirements

Graduate of an accredited Practical Nursing program.
Current Licensed Practical Nurse (LPN) license in the state of practice.
Current American Heart Association BLS Certification.
Phlebotomy experience or demonstrated proficiency in blood collection.
Minimum two years of clinical nursing experience, preferably in primary care, family medicine, concierge medicine, or an outpatient setting.
Strong clinical, communication, and patient service skills.

Nice To Haves

Experience in a clinical setting, particularly in outpatient practices.
Strong clinical assessment and triage skills.
Familiarity with specific EMR systems (e.g., EPIC, Athena).
Experience with patient education and health promotion.

Responsibilities

Build meaningful relationships with a select patient panel.
Work closely with our physician in a true collaborative partnership during patient examinations and procedures, ensuring a comfortable and efficient environment.
Implementation and management of Chronic Care Management (CCM), including accurate documentation, patient enrollment, monthly call compliance, and revenue contribution
Provide direct patient care including vital signs, medication administration, injections, immunizations, wound care, and diagnostic testing within scope of practice.
Manage patient records by accurately inputting information into electronic health records (EHR) and maintaining confidentiality in accordance with HIPAA regulations.
Coordinate comprehensive care plans and follow up with patients personally.
Serve as a primary point of contact, responding to patient needs with care and attention.
Apply expanded clinical skills in a thoughtful, patient-centered environment.
