About The Position

Human Touch Home Health Care is looking for a PRN Licensed Practical Vocational Nurse to join our team in the Colorado area. The Licensed Practical Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. We are a home healthcare provider in the area. We provide comprehensive, skilled, and non-skilled services, to our home bound clients in Colorado, with an excellent track record of client-focused care. We strive to help clients to improve their lives while in the comfort of their homes. To meet our standard of care, we offer a variety of affordable and high-quality home health care solutions that will meet the needs of the seniors, disabled, and ill members of the community.

Requirements

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Has three (3) years nursing experience.
  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
  • Possess and maintain valid CPR certification.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication and organizational skills.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
  • LPN: 2 years (Required)
  • Colorado LPN License (Required)
  • Driver's License (Required)

Nice To Haves

  • Community/home health or medical/surgical experience is preferred.
  • Home health: 1 year (Preferred)

Responsibilities

  • Provides direct patient care as defined in the state Nurse Practice Act.
  • Implements plan of care initiated by the registered nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietitian) to the registered nurse and/or clinical supervisor.
  • Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
  • Participates in personal and professional growth and development.
  • Performs other duties as assigned by the registered nurse.
