About The Position

Rehabilitation Hospital of Oak Creek is hiring a Licensed Practical Nurse for PRN night work at its new 40-bed inpatient rehabilitation facility. The LPN provides patient care and supports rehabilitation patients in maximizing their health, function, and quality of life. The role works with patients and families and collaborates with the interdisciplinary team.

Requirements

• Active Wisconsin or compact-state LPN license; if relocating, licensure must be updated to the new Primary State of Residence before employment begins.
• Associate's degree required.
• AHA Basic Life Support (BLS) certification required within 30 days of hire.
• Critical-thinking skills and decisive judgment.
• Ability to work with minimal supervision.
• Ability to work in a stressful environment and take appropriate action.

Nice To Haves

• One year of previous experience is preferred.

Responsibilities

• Accurately perform patient assessments and identify patient needs.
• Identify and initiate appropriate nursing interventions.
• Provide patient and family teaching throughout the rehabilitation stay and before discharge.
• Provide care appropriate to the patient's condition and age.
• Work cooperatively and actively participate with other members of the interdisciplinary team.
• Participate in Continuous Quality and Performance Improvement (CQPI).
• Complete timely and appropriate medical-record documentation related to medical necessity.
• Complete and revise the Interdisciplinary Care Plan for each patient.
• Perform timely and accurate QI assessments.
